Doctors point out the difficulties in the public network and health plans

In public health, 12,568 bariatric surgeries were performed in 2019, with an increase of 10.2% compared to 2018. So far the 2020 data has only been released by public health. 2,859 surgeries were performed between January and June of this year. In 2019, in the same period, 5,382 such transactions had already been carried out by SUS. According to the SBCBM, the decline is due to the suspension of elective surgeries with the onset of the covid-19 pandemic.
The total number of surgeries performed in 2019 served 0.5% of the severe obesity population, affecting an estimated 13.6 million people in the country, according to the Brazilian Society of Bariatric and Metabolic Surgery. “The only proven effective long-term treatment for obesity and associated diseases, such as diabetes and hypertension, is virtually inaccessible to people who depend on the public system and health plans,” assessed the president of the Company, Marcos Leão Vilas Boas.
According to the organization, the states of Amazonas, Roraima, Amapá, Rondônia and Piauí do not have SUS-enabled bariatric surgery services. Currently, SUS offers 85 highly complex care services for the care of people with obesity in 22 of the 27 federation units.
Metabolic surgery
The agency defends the inclusion of metabolic surgery – indicated for people with diabetes who cannot control the disease with drugs – in the National Agency for Supplemental Health (ANS) procedure list. The procedure, already regulated by the Federal Council of Medicine (CFM), is currently in public consultation with ANS to obtain coverage from health plans.
The Brazilian Society of Bariatric and Metabolic Surgery reported that, in recent years, it has conducted cost and efficacy studies as well as randomized clinical trials demonstrating diabetes remission in patients undergoing metabolic surgery. The organization proposes that surgery, now offered to patients with obesity, is also covered for patients with type 2 diabetes.
“We have diabetes surgery that is fully regulated by a number of resolutions, but this technology is not yet fully accessible to SUS and health plan users. Surgery proves, both in Brazil and abroad, that it is extremely affordable. The budgetary impact is adequate and able to be absorbed by the health system. The industry would have an impact for just ten cents per month and per user, ”said Vilas Boas.
